After 100 years as a specialty, is it not time for us to step up to the plate and establish guidelines in orthodontic treatment? Although there are exceptions to the rules, we do know what works and what does not.
In evaluating such a broad subject as TREATMENT GUIDELINES for long tem stability, one must begin with the understanding that orthodontics will always be part “art” as well as “science.” The patient’s skeletal growth pattern and compliance can be influenced by the orthodontist; however, specific control of these factors is impossible.
The question to be discussed is, “What factors can be controlled to aid in treatment and stability?”
Another issue is not whether or not to extract teeth; rather “where to best place the teeth for esthetics and stability?” Many factors can be addressed that will bring us closer to achieving this stability goal. The research material for this presentation will mostly come from studies of graduate students using the long term diagnostic records of this presenter. (The door is open for future studies.)
Recently, a former patient out of treatment 14 years and out of retention 11 years returned to the office. When checking her original patient chart, T1 (pretreatment) and T2 (post treatment) records were found. In addition, interesting progress photos were also found in her chart. In retrospect several signs in the beginning and finished result indicated that stability could be a potential problem.
This patient’s degree of long term stability will be evaluated based upon evidence-based reasons why these particular results were found. The purpose of this lecture is to define certain goals that should be achieved in order to produce long term stability.
An attempt will be made to define 5 goals in orthodontic treatment that can influence long term stability as well as functional and esthetic results. They will be presented in this order:
(1) Identifying the pretreatment goal.
(2) Presenting evidence of support for that goal,
(3) Discussing mechanics to achieve this goal;
(4) Recognizing exceptions

About Dr. R. G. "Wick" Alexander
Dr. R.G. "Wick" Alexander received his B.A. degree from Texas Tech University in 1958. His dental degrees are from the University of Texas Dental Branch; the D.D.S. in 1962, and the M.S.D. in 1964. Dr. Alexander began the practice of orthodontics in Arlington, Texas, in 1964.
He is a Clinical Professor of Orthodontics, Baylor College of Dentistry, the University of
Texas Dental Branch in Houston, and New York University; Diplomate, American Board of Orthodontics; Fellow, International College of Dentists; and Fellow, American College of Dentists. He is a member of the Angle Society, the Tweed Foundation, and the Southwestern Society of the American Association of Orthodontists.
Dr. Alexander is past president of the University of Texas Orthodontic Alumni Association, the Texas Orthodontic Study Club and the Southwest Component of the Edward G. Angle Society. He served as Trustee to the American Association of Orthodontists representing the Southwestern Society from 1980-82.
In 1966, Dr. Alexander received the Milo Hellman Research Award from the A.A.O. for his study on Scoliosis and the Milwaukee Brace. He also holds both the C.T. Rowland Award and the A.P. Westfall Award (four times) for outstanding orthodontic proficiency and the Fred F. Schudy Award for Excellence in Clinical Orthodontics from the Texas Orthodontic Study Club. In 1987, he received the first Dewel Clinical Orthodontic Award from the American Association of Orthodontists in recognition of his contribution to orthodontic literature. In 1998, he was presented the Dewey Award by the Southwest Society of Orthodontists.
Dr. Alexander is recognized internationally for his innovation in orthodontic procedures. He is well known for his work in designing the Alexander Discipline, a system of Brackets placed on teeth, which is used by orthodontists around the world. He has lectured internationally on the Discipline as well as published many research papers and articles in professional journals. In addition, Dr. Alexander has written a textbook, "The Alexander Discipline", which is used throughout the United Stated and the world. The book has also been translated into several foreign languages.
His second book, The 20 Principles of the Alexander Discipline was published in May 2008, by Quintessence Publishing Co.
In addition to his traveling and teaching, he practices in Arlington with his son, Dr. J. Moody Alexander and lives in Arlington with his wife, Janna. They have three children and nine grandchildren. His sons, Dr. Charles Alexander practices orthodontics in Montrose, Colorado and Dr. J. Moody Alexander practices orthodontics with his father in Arlington. Daughter, Shanna Argote, is married to Luis Argote, general manager of the Four Seasons Hotel in Costa Rica. Shanna is an independent consultant and National Vice President for Arbonne.
