2. Removable Partial Denture Dental Tour Package

The denture tour package includes the creation of a comfortable removable partial denture and a half-day tour of manila within the duration of the tour package. Package also includes panoramic x-ray service and oral prophylaxis.

Procedure

1. Patient must first submit intra-oral digital photos to Enriquez Dental Clinic (hereinafter referred to as “the clinic”) via email before a final quotation and payment instruction can be made.

2. The patient shall then be given a payment instruction based on the quotation and fax a signed conforme with a valid ID indicating agreement to all terms and conditions.  Payment shall be made via wire transfer to the clinic’s bank account.

3. The clinic shall then book all the tours and accommodation after confirmation of the wire transfer.

4. The clinic shall then send a confirmation with voucher numbers for the tour and accommodation to the patient via email. 

Terms and Conditions:

1. The patient’s signature on this form shall be deemed due consent to carry out the requested treatment and all other necessary peripheral treatments on the patient.

2. All prices are subject to change without prior notice.

3. Price not inclusive of roundtrip tickets from your respective country, and back.

4. Patient must first submit intra-oral digital photos to Enriquez Dental Clinic (hereinafter referred to as “the clinic”) via email before a final quotation and payment instruction can be made.  You may send your intra-oral digital photos to info@dentistree.com.  Please indicate desired dental tour package in the body of the email.

5. The clinic also reserves the right to refuse a patient at the last minute for whatever reason.  In which case, a refund commensurate with the cancelled treatment (only) shall be made.

6. In case of a need to extend the duration of the treatment, the additional expense for accommodation shall be borne by the patient.
7. Any unused package component or portion thereof, be it the tour, the accommodation, or the treatment itself shall strictly be non-refundable.
8. While the clinic makes every effort to ensure that all treatment carried out on patients is rendered to the highest possible dental standards, the clinic shall in no circumstances be liable to the patient or to any other person in respect of any claim howsoever arising, whether based on negligence by the clinic or its dentists or otherwise.
